Following the events of Alien vs. Predator, a Predator spaceship is leaving Earth carrying dead Aliens, living facehuggers, and the body of the Predator that defeated the Alien queen. A chestburster erupts from the dead Predator's body; it is a new creature that is a hybrid of Alien and Predator characteristics. It quickly matures into an adult and begins killing Predators throughout the ship. A Predator's weapons fire punctures the hull and the ship crashes in the forest outside of Gunnison, Colorado.

With the Predators dead, the hybrid and several facehuggers escape, implanting embryos into a nearby father and son and into several homeless people living in the sewers. A distress signal from the wrecked ship reaches the Predator home world and a lone Predator responds, traveling to Earth and using its advanced technology to observe the cause of the crash and to track the facehuggers. It begins to erase the evidence of the Aliens' presence by destroying the crashed ship and using a blue liquid to dissolve the bodies of the facehuggers and their victims.

Meanwhile, ex-convict Dallas Howard (Steven Pasquale) has just returned to Gunnison after serving time in prison. He is greeted by Sheriff Eddie Morales (John Ortiz) and reunites with his younger brother Ricky (Johnny Lewis). Ricky has a romantic interest in his more affluent classmate Jesse (Kristen Hager) and is being harassed by her boyfriend Dale (David Paetkau) and two of his friends. Kelly O'Brien (Reiko Aylesworth) has also just returned to Gunnison after service in the military, and reunites with her husband Tim (Sam Trammell) and daughter Molly (Ariel Gade).

The Predator fights a number of Aliens in the sewers, and as the battle reaches the surface several of them disperse into the town. The Predator pursues some to the power plant, where collateral damage from its weaponry causes a city-wide power outage. Ricky and Jesse meet at the high school swimming pool but are interrupted by Dale and his cohorts just as the power fails and an Alien enters the building, killing Dale's friends. Another Alien invades the O'Brien home, killing Tim while Kelly escapes with Molly.

Kelly, Molly, Ricky, Jesse, Dale, Dallas, and Sheriff Morales meet at a sporting goods store to gather weapons. Troops from the Colorado Army National Guard arrive but are quickly killed by the Aliens. When the battle between the Predator and the Aliens enters the store, Dale is killed and the Predator's shoulder cannons are damaged; it is able to modify one into a hand-held blaster.

As the survivors attempt to escape Gunnison they make radio contact with Colonel Stevens (Robert Joy), who indicates that an air evacuation is being staged at the center of town. Kelly is suspicious of the military's intentions, convincing a small group to go to the hospital where they hope to escape by helicopter, while Sheriff Morales heads to the evacuation area with the rest of the surviving citizens. The hospital, however, has been invaded by Aliens and the hybrid creature. The Predator soon arrives and in the ensuing battle Jesse is killed, Ricky is injured, and Dallas takes possession of the Predator's blaster cannon.

As the battle reaches the rooftop, Dallas, Ricky, Kelly, and Molly escape in the helicopter while the Predator battles the hybrid hand-to-hand. The two creatures mortally wound each other just as a military jet arrives; rather than a rescue airlift it is a bomber, executing a tactical nuclear strike that destroys the entire city and kills all of the extraterrestrials along with the remaining citizens. The shock wave causes the fleeing helicopter to crash in a clearing, where the survivors are rescued by the military. The Predator's blaster cannon is confiscated by Colonel Stevens and presented to a Ms. Yutani.

Fisherman Matthew Boyd (Richard Dreyfuss) is fishing in Lake Victoria, Arizona when a small earthquake hits, splitting the lake floor and causing a whirlpool. Boyd falls in and is ripped apart by a school of piranhas that emerge from the chasm and ascend the vortex.

Jake Foster (Steven R. McQueen) is admiring attractive tourists as spring break begins. He reunites with his old crush, Kelly (Jessica Szohr) and meets Derrick Jones (Jerry O'Connell), an eccentric pornographer, as well as Danni (Kelly Brook), one of his actresses. Derrick convinces Jake to show him good spots on the beach for filming a pornographic movie.

That night, Jake's mother, Sheriff Julie Forester (Elisabeth Shue), searches for the missing Matthew Boyd with Deputy Fallon (Ving Rhames). They find his mutilated body and contemplate closing the beach down, made difficult by 20,000 partying college students. The next morning, a lone cliff diver is attacked and consumed by the marauding fish.

Jake bribes his sister and brother, Laura (Brooklynn Proulx) and Zane (Sage Ryan), to stay home alone so that he can show Derrick around the beach. After Jake leaves, Zane drafts Laura to go fishing on a small sandbar island. They forget to tie the boat down and are stranded in the middle of the lake. Meanwhile, Jake goes to meet with Derrick and runs into Kelly, who invites herself onto Derrick’s boat, The Barracuda. Jake meets Crystal (Riley Steele), another of Derrick’s actresses, and cameraman Drew (Paul Scheer).

Julie takes a team of seismologist divers — Novak (Adam Scott), Sam (Ricardo Chavira), and Paula (Dina Meyer) — to the fissure. Novak speculates that the rift leads to a buried prehistoric lake. Paula and Sam scuba dive to the bottom and discover a large cavern filled with large piranha egg stocks. Both are killed by the piranhas before they can alert the others to the discovery. Novak and Julie find Paula's corpse and pull it onto the boat, capturing a lone piranha. They take the fish to Carl Goodman (Christopher Lloyd), a marine biologist who works as a pet store owner. He explains that the piranha that they caught is a prehistoric species, long believed to be extinct, which must have been trapped underground for over two million years where they have grown super-aggressive.

Julie, Novak, Fallon, and Deputy Taylor Roberts (Jason Spisak) try to evacuate the beach, but their warnings are ignored until the piranhas begin to attack the tourists. Novak boards a jet-ski with a shotgun to help while Fallon drags people to shore and Julie and Taylor try to get swimmers into the police boat. A floating stage capsizes from the weight of all the panicking guests and the wet T-shirt contest host (Eli Roth) is killed.

Meanwhile, Jake spots Laura and Zane on the island, and forces Derrick to rescue them. Derrick crashes the boat into some rocks, flooding the rooms below deck. Kelly is trapped in the kitchen while Derrick, Crystal, and Drew are thrown from the boat. Crystal is devoured and Drew disappears and is presumed dead. Meanwhile, Danni manages to get a partially eaten Derrick back on board.

Jake calls his mother for help. Julie and Novak commandeer a boat and take it to the sinking Barracuda, while Fallon stays behind to fight off the piranhas.

Julie and Novak reach Jake and attach a rope to his boat. Julie, Danni, Laura, and Zane start crossing the rope, but the piranhas latch onto Danni's hair and ultimately devour her. The others make it to safety, but the rope comes loose. Using Derrick's corpse as a distraction, Jake ties the line to himself and goes to save Kelly. He ties Kelly to him and lights a flare after releasing the gas in a pair of stored propane tanks. Novak starts the boat and speeds away just as the piranhas surround Kelly and Jake. They are dragged to safety and the propane tanks explode, destroying the boat and killing most of the piranhas.

Mr. Goodman calls Julie on the radio, and Julie tells him that they seem to have killed the majority of the piranhas. Terrified, Goodman tells her that the reproductive glands on the piranha they obtained were not mature, which means that the fish they were fighting were not adults. As Novak wonders aloud where the parents are, a huge piranha leaps out of the water and drags him into the water.

The film begins with a voice over from a man named Ramirez who tells the viewers of stories that his mother had told him as a child that sometimes the Devil actively seeks out individuals who have sinned on Earth, and while taking human form he traps these individuals in a confined place only to turn them against each other, to kill them off one at a time. He further explains that the last one to die will die in front of his or her loved one to make cynics of them all, and in the end all will die. He says that the signs are set in motion with a suicide, as that is when the Devil first makes his presence known. This story is abruptly interrupted by a violent suicide of a man who had fallen from the thirty-fifth floor of an office building, causing the truck he landed on to roll silently away from the scene.

Detective Bowden (Chris Messina) is a recovering alcoholic who is assigned to the case of the suicide. He later explains that the reason for his alcoholism was the hit and run death of his wife and toddler son five years prior. When he investigates the suicide, he determines that the truck originated from an office building down the block from where the victim was found. Meanwhile, five strangers board an elevator, which becomes stuck between floors just shortly after starting up to the upper floors. The five don't introduce themselves right away, but when security finds them, they notice that other than the CCTV and a radio which they can communicate into the elevator, they have no way of hearing the passengers in return. Bowden is in the area at the time of the call about the elevators and takes the investigation as it is the same building from which suicide had been committed. Ramirez is revealed to be one of the security guards and is disturbed by an image of what looks like a screaming face frozen on the video monitor that flashes away. His boss Lustig (Matt Craven) dismisses the superstitious suspicions and sends repair technician Dwight to investigate the elevators while Bowden tries to ascertain the identities of the individuals. Only four of the five are accounted for: Vince McCormick (Geoffrey Arend), a sleazy mattress salesman known for investments and frauds; Sarah Caraway (Bojana Novakovic), the wife of a rich man who is a pathological liar and plans to leave her husband and take his money; Ben Larson (Bokeem Woodbine), a temporary security guard with a history of violence; Jane Kowski (Jenny O'Hara), an older woman who is seen on video cameras to be a thief, and another man who later introduces himself as Tony (Logan Marshall-Green), who does not appear to have signed in at the security desk.

Things turn from strange to frightening when there is a freak power outage in the elevator, and Sarah is wounded with what appears to be a bite. All suspicion is turned on Vince who has been dodgy since the ride began, however there is another power failure and Vince is thrown into the mirror only to have his jugular vein sliced, the others watch in horror as he writhes and dies, forcing Bowden to consider this a murder scene. Dwight attempts to rappel down the elevator shaft via a pulley to try and fix the elevator, but Ramirez says that the Devil would stop any attempts to help his victims, and Dwight's strap comes loose when he is knocked over by a flock of birds and plummets to the roof of the elevator. The power goes out again. When it comes back, Jane is found hanging high off the floor, dead, causing both Sarah and Ben to accuse Tony of these malicious acts while Tony swears his innocence. Lustig goes to investigate the power in the basement while Ramirez tells Bowden about the story of the Devil incarnate on Earth. Lustig finds a loose power cable that appears to be what sabotaged the elevators, but he is electrocuted trying to fix the issue. Bowden finds a set of tools that Tony had brought in and suspects he might be responsible for the elevator damage. However, he wonders if Ben might have been possibly hired to kill Sarah by her husband to keep her from leaving him broke. However Ben is the victim of the next blackout, his neck is twisted around grotesquely. Sarah and Tony face off with each other, but decide to lower their weapons after Detective Bowden calms them down over the loud speaker. Then there is a sudden blackout and Sarah's throat is cut. As she dies in his arms, a woman claiming to be Tony's fiancee reveals that Tony's last name was "Jankowski," thus eliminating the possibility of the old woman's name being Jane Kowski.

As they realize this the old woman rises up, revealing herself to be the Devil. Tony tries to bargain with her to save Sarah's life and sacrifice his own; she scoffs at this, having revealed she'd kill him anyway. In a last, desperate plea for forgiveness, Tony, using a radio, confesses to a hit and run five years earlier - while drunkenly reaching for beer in his car, he smashed into another car and killed a woman and her child. Bowden realizes that Ramirez was right having being meant to be the audience to this. The Devil, powerless now that Tony has repented of his sin, curses before vanishing, and the elevator comes back on line, leaving Tony's life spared. Bowden offers to take Tony into custody for the confession, and while en route, informs him who he is. But despite saying he'd gone over what he'd say, or what he would do if they'd ever cross paths, he forgives Tony.

The film ends with Ramirez, in a voice-over, telling the audience that his mother always reassured him at the end of her stories, "If the Devil is real, then God must be real too."

Dean Russell (Kellan Lutz) goes to the Springwood Diner with his girlfriend, Kris Fowles (Katie Cassidy), falls asleep at the table and meets a man covered in burn scars, wearing a red and green sweater and a clawed glove on his hand. The burned man cuts Dean's throat in the dream, but in reality it appears that Dean is cutting his own throat as his friend, waitress Nancy Holbrook (Rooney Mara), looks on with Kris. At Dean's funeral, Kris sees a photograph of her and Dean as children, but cannot recall ever knowing Dean before high school. Kris begins to dream about the burned man herself and refuses to go to sleep for fear that she will die in her dreams. Jesse Braun (Thomas Dekker), Kris's ex-boyfriend, shows up at her house to keep her company while she sleeps, but Kris meets the burned man in her dreams and is murdered. Covered in blood, Jesse runs to Nancy's house to try to explain what happened and he learns that Nancy has been having dreams about the same man: Freddy Krueger (Jackie Earle Haley).

Jesse is apprehended by the police under suspicion of murdering Kris, and is killed by Krueger when he falls asleep in his jail cell. With her friends dying, Nancy begins to question what everyone's connection is to each other, given that none of them can remember each other before their teenage years. Eventually, Nancy and her friend Quentin Smith (Kyle Gallner) discover that all of them attended the same preschool together. Nancy's mother, Gwen (Connie Britton), reluctantly tells Nancy and Quentin that there was a gardener at the preschool, Fred Krueger, who abused Nancy and the rest of the kids. Gwen explains that Nancy was his favorite, and came home one day telling her mom about a hidden space in Krueger's room and the things he did to her there. Gwen claims Krueger skipped town before he was arrested. Nancy does not believe her and attempts to track down the remaining kids from the school. Nancy eventually discovers that all of the other kids have been killed, most of them in their sleep. Meanwhile, Quentin tries to accept that his nightmares are nothing more than repressed memories, but he falls asleep during swim practice and witnesses what really happened to Krueger. Quentin sees everyone's parents hunt down Krueger, and then burn him alive. Quentin and Nancy confront Quentin's father, Alan Smith (Clancy Brown), about the reality that they murdered Krueger with no evidence that he had committed any crime. Thus, Nancy and Quentin believe that Krueger wants revenge on them for lying as children. As result of their insomnia, Nancy and Quentin begin sporadically dreaming while they are still awake. To try and stop Krueger, they decide to go to the preschool and learn what they can.

On the way, Nancy falls asleep and is attacked by Krueger, but when Quentin wakes her up they discover she has pulled a piece of Krueger's sweater out of the dreamworld and into reality. Quentin takes Nancy to the hospital for cuts on her arm; there, he steals some adrenaline and a syringe to help them stay awake. Nancy and Quentin eventually make it to the preschool. Quentin uncovers Krueger's hidden room and the evidence that proves Krueger was in fact abusing all of the children; they realize that Krueger actually wants revenge on them for telling the truth. Nancy decides the only way to end this is to pull Krueger out of their dreams and kill him in reality. Quentin tries to stay awake long enough to pull Nancy out of her dream when she has Krueger, but he falls asleep and is attacked. Krueger then goes after Nancy, and explains that he intentionally left her for last so she would stay awake long enough that, when she finally fell asleep, she would no longer be able to wake up. While Nancy struggles with Krueger, Quentin uses the adrenaline to wake up Nancy who pulls Krueger into reality. With Krueger distracted by Quentin, Nancy uses a broken paper cutter blade to cut Krueger's gloved hand off, and then slice his throat. Afterward, Nancy torches the secret room, with Krueger's body left inside, while she and Quentin leave. Nancy and her mother return home from the hospital; Krueger suddenly appears in a mirror's reflection and kills Nancy's mother before pulling her body through the mirror while Nancy screams.

In 2019, a plague has transformed most of the world's human population into vampires. As human numbers dwindle, the need for blood becomes desperate. When deprived of blood for extended periods, vampires degenerate into subsiders: aggressive bat-like creatures. Humans are captured and harvested in laboratory farms while scientists research a synthetic blood substitute to satisfy vampires' blood hunger. The main supplier of blood in the United States is the pharmaceutical company Bromley Marks, run by Charles Bromley (Sam Neill). Edward Dalton (Ethan Hawke) and Chris Caruso (Vince Colosimo) are hematologists working on the blood substitute. Under pressure from Charles, they try multiple versions in a series of failing clinical trials. Edward secretly refuses to drink human blood, and faces a strained relationship with his brother, Frankie (Michael Dorman), a human-hunting soldier.

Due to his refusal to drink human blood, Edward begins to notice signs that he is becoming a subsider. While driving home from work he notices his ears are becoming pointed (a key indicator) by looking at himself in the vanity mirror. Distracted, he almost collides with another car, which swerves to avoid him and hits a power substation. Edward exits his car to check on the passengers of the other vehicle, who we find are a group of humans led by Audrey (Claudia Karvan), who rescues and hides humans from vampire patrols while they attempt to win allies amongst the vampires. Edward lets the group hide in his car as the police arrive, and tells the officers that they ran off in another direction. Audrey then leads the group of humans away, but not before stealing Edward's ID badge. Later, Audrey appears at Edward's home and asks for his help, presumably because of his human sympathies and scientific specialty. He meets Elvis (Willem Dafoe), a man cured of vampirism after a car crash resulted in him being exposed to sunlight before he fell into a river. After some experimentation, Edward is able to duplicate this effect and is cured himself.

When a convoy of humans headed to Audrey's group is captured by Bromley soldiers, Charles is reunited with his human daughter, Alison (Isabel Lucas), and has her forcibly turned by Frankie. She refuses to drink human blood, but feeds on her own instead - and thus begins to turn into a subsider. She is killed when a group of the creatures are rounded up and executed. Witnessing Alison's death upsets Frankie and he seeks out his brother.

Edward, Elvis, and Audrey meet with Chris and ask him to help spread the cure. However, Chris has finally discovered a viable blood substitute and does not want a cure to become widespread. He calls in a vampire patrol which captures Audrey while Elvis and Edward escape into hiding. They are finally found by Frankie, who agrees to help but is gradually becoming a subsider. He attacks Elvis and they discover that feeding on a former vampire is another cure for vampirism.

Edward turns himself in and goads Charles into attacking him. Edward uses the now-cured Charles to cure a group of soldiers experiencing the beginning of subsider aggression. Edward and Audrey, cornered by more soldiers, are rescued when Frankie sacrifices himself to the soldiers. This sparks a feeding frenzy that leaves all of the soldiers dead or cured. They are confronted by Chris, who kills the few remaining cured soldiers to hide the evidence of the cure. Elvis arrives and rescues Edward and Audrey by killing Chris. The three survivors then drive away into the sunrise with a voice-over by Edward stating that they have a cure and can change others back.

The film opens as a well dressed, barrel chested man stalks the late-night passengers of a subway train. He kills several people with a meat hammer and a butcher's hook. He dispatches his prey with unnatural strength. He wears a ring on his finger, adorned with an eight-pointed star. The film then introduces Leon (Bradley Cooper), a photographer who heads into the city's subway system at night to take photographs, and saves a woman from a group of guys harassing her. The next day, he discovers the girl has gone missing. Leon is intrigued and begins to investigate newsreels about similar disappearances. His investigation leads him to a butcher named Mahogany (Vinnie Jones), whom he suspects has been killing subway passengers for over a hundred years.

Leon attempts to turn some of the photos he has taken of Mahogany in to the police, but they become suspicious of Leon's motives. Leon decides to take matters into his own hands and goes to the subway at midnight, where he witnesses the butcher killing several passengers and hanging them on meat hooks. Leon passes out on the subway floor after being attacked by the butcher who then strings him up like the other dead bodies. He passes out again when "something" begins to attack him and awakens the next morning in an abandoned subway platform with strange markings carved into his chest. Maya (Leslie Bibb), Leon's girlfriend, and her friend Jurgis (Roger Bart) examine the photos Leon has been taking of Mahogany, leading them to the killer's apartment. After breaking into the butcher's home, Jurgis is kidnapped. Maya goes to the police, but they won't listen to her. A police official, who seems to be involved with the unfolding conspiracy, directs the misguided Maya to a trip on the midnight train. Leon, unaware of Maya's involvement, decides to put an end to the butcher's crimes and heads to the meat packing plant Mahogany works in. Leon arms himself with a butcher's apron and several slaughterhouse knives.

Leon enters the train as Mahogany corners Maya. Leon attacks Mahogany and finally throws him out of the train. Eventually, they enter an underground cavern filled with bones and decomposing bodies. The train engineer enters the car and advises Leon and Maya to "Please, step away from the meat." The true purpose of the underground station is revealed as reptile-people enter the car and consume the bodies of the butcher's victims. As they exit the train, Leon and Maya discover they are in a station underneath city hall which is littered with the bones of hundreds of victims. Mahogany returns in a battered and bleeding state and charges at him, and Maya is tossed aside. Leon engages him a final time ending with Leon driving a sharpened bone through Mahogany's throat. Mahogany whispers with his dying breath, "Welcome," at which point Leon impales Mahogany's skull with a butcher knife. The train engineer explains to Leon that the reptile-people have always existed and the butcher's job is to feed them every night, in order to keep "the worlds separate". The engineer rips out Leon's tongue with the same superhuman strength as the butcher and eats it; then he forces Leon to watch as he kills Maya on an altar of corpses with one of the butcher's knives and rips out her still-beating heart. When the engineer is done, he tells Leon that he will become the new meat train butcher, replacing Mahogany.

In the final scene, the police official who was involved hands the train schedule to the new butcher, who wears a ring with the same eight-pointed star. The killer walks onto the midnight train and turns to reveal that he is a now mute Leon.

In a dark, dungeon-like room, a young woman tries to call for help on her mobile phone. Unable to receive a signal, she is dragged away by an unseen force.

Two young American couples (Jeff and Amy along with Eric and Stacy) vacation in Mexico. At the pool of their hotel, they make friends with a German tourist, Mathias, and decide to help Mathias look for his brother, Heinrich. Heinrich has met a female archaeologist and followed her to an archaeological dig at a remote Mayan ruin in the jungle. The next morning Jeff, Amy, Eric, Stacy, Mathias, and Demetri and Gregor (a pair of greek tourists they also met at the hotel) begin the journey by bus.

The location of the ruin is shown on a crude map Heinrich drew before departing. After a long ride, the group arrives at a remote village near the dig and takes a worn-down taxi to the destination marked on the map. The driver tries to warn them away from the area, but they insist, and after arriving at the trail head, they spot Heinrich's jeep and proceed.

Upon arrivial, Mayan villagers appear with guns and bows. When Amy accidentally steps into vines covering the ruin, the Mayans aim their weapons at them. Demetri tries to calm the situation by walking over to Amy and grabbing the camera, which he thought was the cause of the problem. After stepping into the vines and taking the camera he begins to walk towards the Mayans. Then he is struck with an arrow in his shoulder and then shot in the head. The Mayans then force the others up to the temple roof.

They find an abandoned camp with two tents and a shaft in the center leading down vertically into a temple. Eric and Mathias are the only two with mobile phones, but Eric's has no signal and the Mayans stole the other. At the shaft bottom, they can hear a mobile phone ringing. Mathias says he is certain it's his brother's ringtone and is lowered down. But the rope breaks, and he falls into the darkness, hitting bottom thirty feet below. Meanwhile, Amy descends the temple steps to reason with the Mayans. When she angrily throws a clump of vines at them and hits a young boy, they promptly kill the boy. Later, Stacy and Amy also descend the shaft to help Mathias and to find the mobile phone. Mathias has broken his back. Jeff and Eric rig a backboard and the paralyzed Mathias is lifted out.

Next morning, Stacy observes a tendril of vine has crept into a wound on her leg. The vines have also wrapped themselves around Mathias's lower legs and eaten them down to the bone. The mobile phone is heard once more. Stacy and Amy descend the shaft and in a small, vine-covered room find the body of the young archaeologist from the opening scene and her phone, which is broken. The flowers are vibrating and screeching, reproducing the ring of the phone. The vines attack, but the two manage to escape. Stacy is convinced the vines are alive, and growing inside her. The group now realizes the vines are predatory and the Mayans are keeping everyone quarantined. They have also salted the earth all around the hill. Jeff insists that "four Americans on vacation don't just disappear" and that help is on the way.

With Mathias' condition getting worse, Jeff amputates both his legs. Stacy becomes jealous at Eric comforting Amy, saying she overheard them having sex. When Jeff tries to defuse the situation, Stacy calls Amy's fidelity into question. While the four argue, the vines suffocate Mathias. The group suddenly realizes that the vines and their flowers are able to replicate their sounds.

Stacy knows the vines are now growing inside her and can be seen moving across her back, leg, and forehead. Jeff cuts out two but stops after realizing the futility of his efforts. Stacy claims she can feel more, but the others reluctantly deny her pleas to remove them. While the rest sleep, Stacy sneaks out of her tent and begins to cut the other vines out herself. When Eric tries to stop her, she fatally stabs him. As the vines move in to take Eric's body, Jeff removes the knife and kills Stacy, who is crying to be put out of her misery.

Realizing they'll eventually die, Jeff makes a plan for Amy to escape. He carries her to the bottom of the temple and lays her on the ground making it seem she has died. While distracting the Mayans, Jeff is shot with an arrow, then shortly later dispatched with a shot to the head. Meanwhile, Amy makes it to the jeep and escapes, but before the film cuts to black, the vines can be seen moving under her skin. In a final scene, Demetri's Greek friends are seen moving toward the temple, repeating the cycle.

Police officer Frank Williams (Steven Vidler) and his partner Blaine enter an abandoned house, only to find a young woman held hostage, her eyes ripped out. Suddenly, Blaine is savagely murdered by an unknown figure with an axe, before Frank is attacked too. Frank manages to fight off the man, but his arm is severed in the process. In the aftermath, detectives find seven bodies in the house, all of which have had their eyes ripped out.

Four years later, Frank is taking a group of delinquents -Russell Wolf (Mikhael Wilder), Richie Bernson (Craig Horner), Christine Zarate (Christina Vidal), Zoe Warner (Rachael Taylor), Michael Montross (Luke Pegler), Kira Vanning (Samantha Noble), Tyson Simmons (Michael J. Pagan) and Melissa Beudroux (Penny McNamee)-, to clean up the abandoned Blackwell Hotel in order to turn it into a homeless shelter, as explained by the owner Margaret (Cecily Polson). As the delinquents work, they do not realize that reclusive psychopath, Jacob Goodnight (Kane) has holed away in the rotting hotel.

Tyson and Richie decide to look for the previous owner's safe, which was never recovered after the fire that broke out in the hotel, while Kira deals with being reunited with Michael, her abusive ex-boyfriend who was the one who sent her to jail. While searching for the safe, Tyson and Richie discover a dead body. Richie freaks out and attempts to leave, but is attacked by Goodnight, who drags him into an elevator, in front of a terrified Tyson.

Margaret realizes the elevator is being used, and informs Frank's partner, Hannah (Tiffany Lamb), who goes to check if everyone is in their rooms. However, the elevator does not stop and takes Hannah up into the damaged floors of the hotel, where she is swiftly killed by Goodnight, who rips her eyes out. Meanwhile, Christine agrees to help Kira escape the hotel after catching Michael hurting her in the bathroom. While Kira is escaping, she is attacked by Goodnight, who drags her into the dumbwaiter. Christine, Frank and Margaret arrive just as she is assaulted, and Christine informs Frank the others have gone to the restricted penthouses to have a party. While Margaret stays behind to phone the police, Christine and Frank enter the elevator, where they find Hannah's blood, before bumping into Tyson, who warns them of the man who took Richie. Frank realizes it is Goodnight, before a hook goes through his chin and drags him away, dumping his body in front of Tyson and Christine.

Kira is held hostage by Goodnight, due to her religious tattoos. She awakens in Goodnight's lair, where Richie has been tied up. Goodnight soon appears and gouges Richie's eyes out, before we see a flashback to Goodnight's childhood (being locked in a cage, drowning in the shower, and watching a strapped woman have her eyes removed by his mother), suggesting his killing sprees are owed to religious abuse.

Meanwhile, Zoe, Michael, Russell and Melissa are taking drugs in a penthouse. Soon after, Russell and Melissa leave for some privacy. As the pair make out, they are attacked by Goodnight. They manage to escape and hide in a room, where they realize they can climb out the window to get to the ground level of the hotel. Russell lowers Melissa out of the window by a rope, but Russell is stabbed to death by Goodnight, who then lets Melissa fall. The rope stops her head from hitting the ground, but she hurts her hand on impact, leading a group of stray dogs to devour her.

Zoe and Michael become sick of each other and attempt to go back to their rooms, but are attacked by Goodnight. They hide, but Zoe is found when her cell phone goes off. She pleads for help, but Goodnight shoves the phone down her throat, killing her. Christine and Tyson decide to stay and try to find Kira, and bump into Michael. Goodnight finds them, and Christine and Tyson escape through the elevator shaft, forgetting about Michael. who is violently beaten up by Goodnight.

Christine and Tyson find Goodnight's lair and create a distraction to free Kira. As Christine tries to open Kira's cage, Tyson attempts to fight off Goodnight, but he gets electrocuted by his taser before being crushed with a vault. Christine hides as Goodnight returns. Margaret turns up and unexpectedly reveals herself to be the mother of Goodnight. Margaret tells Kira that she lured Frank back to the hotel to get revenge on him for hurting her son four years before, and that the prisoners all had to die because they were full of sin, including her. Margaret attempts to shoot Kira, but Goodnight kills her by accident, throwing her into a large nail that impales her.

Goodnight becomes enraged by his mother's death, and is about to kill Kira when Christine attempts to help. Goodnight grabs both Kira's and Christine's throats, but throws Kira away and attempts to gouge out Christine's eye. Michael appears and strikes from behind, giving the girls time to hide.

Goodnight searches the room they have hidden in, but the three of them catch him by surprise, sending him out the window to fall to his death. The three survivors escape to freedom, and the credits roll, only to be interrupted by a dog that urinates in Goodnight's empty eye socket.

College student Alex and her friends go into the woods on a rafting trip. Her friends are killed by the mutant cannibal 'Three Fingers', from the previous films. She is left to fend for herself, but not for long. Two days later a most dangerous Mexican prisoner, Carlo Chavez, and group of prisoners are being transferred by Nate Wilson, a prison guard, on his last day before leaving to become a lawyer. On the way, Nate meets up with an old friend, Sheriff Calvin Carver, making viewers aware that Nate used to live in these lands. During transportation their bus is attacked by Three Fingers and they escape.While being chased through the woods, the prisoners led by Chavez, use Nate as a guide to lead them to a watch tower (which was the tower from the first movie) to issue an SOS. Officer Walter, now a prisoner, encounters Alex and an abandoned armored truck filled with cash. The money quickly becomes the prisoners' sole motivation for escape and survival. Chavez commands Walter to pick up one of the bags of money, but Walter refuses because of his injury from Three Fingers. Walter pulls out a gun and tries to shoot Chavez. Unfortunately, the gun was not loaded and Chavez kills Nate's friend.

During the trail, they stumble upon a booby trap. The prisoners escape the booby trap set by Three-Toes (Three Finger's son from the previous film). They then catch the son and behead him, leaving a trophy on the spent booby trap for Three Fingers to find as a message. They soon realize it only made Three Fingers even more fearsome. One-by-one, Three Fingers hunts and murders every prisoner and kills the sheriff and his deputy. During the killings, Nate and Alex manage to escape. They are able to find the watch tower, which was badly burned years ago (first movie). Chavez decides to get rid of Nate, but instead promises him another way to get out. They find Three Finger's truck and Chavez uses Alex as bait. Three Fingers kidnapped Alex and Nate rescues her. Floyd steals the money and Chavez chases him down only to see his money burning and Floyd gets killed by Three Fingers. Three Fingers goes to another location to find the rest, until he is face to face with Chavez and battles him with a Hatchet and a meat hook. Both beat one another, but Three Fingers eventually kills Chavez, hangs his corpse in a tree, removes his scalp and eats part of his brain. Nate finds Three Fingers' house with the help of the sheriff's dog and frees Alex. Three Fingers hides behind a door and ambushes Nate. The dog attacks Three Fingers to save them, but is killed by the hook. Alex stabs Three Fingers using the large stake that held Three Toes' head and believes she killed him. Nate and Alex take his tow truck and drive away. Thinking it is over, Three Fingers follows them and gets on top of the truck. Nate crashes into a tree and Brandon the prisoner, sees the whole thing and decides to help them. The truck catches fire and is about to explode, when Brandon gets Alex out then Nate. Three Fingers attempts to attack them but something goes wrong and he finally appears to be killed when Nate stabs him in the head with his own meat hook.

Nate lets Brandon go freely as he had not actually murdered the bartender (the crime that he was convicted for) & tells him that police will not go looking for a dead con, but asks him to stay out of trouble. Brandon promises to do so & leaves. Soon after the U.S. Marshall team arrives, rescues Nate & Alex and escort them to safety.

Sometime later, Nate returns to collect the remaining cash from the truck (the rest having been burned by "Three Fingers" earlier). But Brandon, who is one of the convicts that Nate let go, kills him with an arrow through the chest, with the retort, "Never trust a con". Unknown to Brandon, Three Fingers, having survived, appears from behind holding a crude weapon dripping with blood, he walks towards Brandon and Brandon's scream is heard over the end credits.

In a short flashback, Deborah Myers (Sheri Moon Zombie) visits her son, a young Michael Myers (Chase Wright Vanek), at Smith's Grove Sanitarium. Deborah gives Michael a white horse statuette as a gift. Michael says that the horse reminds him of a dream he had of Deborah's ghost, all dressed in white and leading a horse down the sanitarium halls toward Michael, telling him she was going to bring him home. Moving ahead fifteen years, after having shot an adult Michael (Tyler Mane), Laurie Strode (Scout Taylor-Compton) is found wandering around in a state of shock and covered in blood by Sheriff Brackett (Brad Dourif). Brackett takes Laurie to the emergency room. Meanwhile, the paramedics pick up the Sheriff's daughter Annie (Danielle Harris) and Michael's psychiatrist Dr. Loomis (Malcolm McDowell), who are still alive after having been attacked by Michael, and take them to the hospital. Presumed dead, Michael's lifeless body is loaded into a separate ambulance. When the driver has a traffic accident Michael awakens and escapes the ambulance, walking toward a vision of Deborah dressed in white and leading a white horse.

Michael appears at the hospital, and begins murdering everyone he comes across on his way to Laurie. Trapped in a security outpost at the gate, Laurie watches as Michael tears through the walls with an axe, but just as he tries to kill her, Laurie wakes up from the dream. It is actually one year later and Laurie is now living with the Bracketts. Michael's body has been missing since last Halloween—still presumed dead—and Laurie has been having recurring nightmares about the event. While Laurie deals with her trauma through therapy, Loomis has chosen to turn the event into an opportunity to write another book. Meanwhile, Michael has been having visions of Deborah's ghost and a younger version of himself, who instructs him that with Halloween approaching it is time to bring Laurie home; so he sets off for Haddonfield.

As Michael travels to Haddonfield, Laurie begins having hallucinations that mirror Michael's, which involve a ghostly image of Deborah and a young Michael in a clown costume. In addition, her hallucinations also begin to include her acting out Michael's murders, like envisioning herself taping Annie to a chair and slitting her throat while dressed in a clown outfit—similar to how a young Michael murdered Ronnie White. While Laurie struggles with her dreams, Loomis has been going on tour to promote his new book, only to be greeted with criticism from people who blame him for Michael's actions and for exploiting the deaths of Michael's victims. When his book is finally released, Laurie discovers the truth: that she is really Angel Myers, Michael's long lost sister. With the truth out, she decides to go partying with her friends Mya (Brea Grant) and Harley (Angela Trimbur) to escape how she is feeling. Michael appears at the party and kills Harley, then makes his way over to the Brackett house and stabs Annie repeatedly. When Laurie and Mya arrive they find Annie bloodied and dying. Michael kills Mya and then comes after Laurie, who manages to escape the house. While Laurie manages to flag down a passing motorist, Sheriff Brackett arrives home and finds his daughter dead. Laurie gets into the motorist's car, but before they can escape Michael kills the driver and flips the car over with Laurie still in it. Michael takes the unconscious Laurie to an abandoned shed he has been camping out in. Laurie awakens to a vision of Deborah, and a young Michael, ordering her to say "I love you, mommy".

The police discover Michael's location and surround the shed. Loomis arrives and goes into the shed to try to reason Michael into letting Laurie go. Inside, he has to inform Laurie, who believes that the younger Michael is holding her down, that no one is restraining her and that she must maintain her sanity. Just then, Deborah instructs the older Michael that it is time to go home, and Michael grabs Loomis and begins repeatedly slashing his face and stabbing him in the chest. Stepping in front of a window while holding Loomis's body, Michael is shot twice by Sheriff Brackett and falls onto the spikes of some farming equipment. Apparently released of the visions, Laurie walks over and tells Michael she loves him, then she stabs him repeatedly in the chest and finally in the face. The shed door opens and Laurie walks out, wearing Michael's mask. As she pulls the mask off, the scene transitions to Laurie in isolation in a psychiatric ward, grinning as a vision of Deborah dressed in white stands with a white horse at the end of her room.

On August 7, 1939, after a mother dies giving birth to a child in a meat-packing plant in Travis County, Texas, the plant manager abandons the newborn in a dumpster. A tramp, Luda Mae Hewitt, discovers the baby while searching for food. She names the infant Thomas and takes him to the Hewitt residence.

In July 1969, Thomas grows up and goes to work in the same slaughterhouse, working for the same boss who put him in the dumpster. The plant is closed by the health inspector due to its horrid conditions. When the boss insults Thomas calling him an "animal", he leaves. Later that day Thomas returns and brutally mauls his boss with a sledgehammer, smashing his skull into his brain. While leaving he finds and takes a chainsaw. Charlie Hewitt (R. Lee Ermey), Luda Mae's son, learns from Sheriff Hoyt what Thomas has done, and accompanies the Sheriff to arrest Thomas. When they find Thomas, Charlie kills the Sheriff with a shotgun and takes his identity. Charlie and Thomas take the Sheriff's body home and butcher him for stew meat, telling the rest of the family that they will not leave and with "good people like the sheriff here" they'll never go hungry again.

Meanwhile two brothers, Eric (Matthew Bomer) and Dean (Taylor Handley), drive across the country with their girlfriends Chrissie (Jordana Brewster) and Bailey (Diora Baird) They are traveling to enlist to go to Vietnam, Eric having already been, and Dean with a draft card. They stop at a local eatery where they encounter a group of bikers. As the group pulls away, a female biker, Alex, follows them. Dean decides he's not going to Vietnam and starts to burn his draft card in front of Eric. Alex pulls a shotgun out to get the group to pull over. A chase ensues, ending with the group crashing into a cow and flipping their car when Eric turns around to shoot at the biker. In the process, Chrissie gets thrown from the car and lands in the weeds, out of sight of the others. Charlie Hewitt/Sheriff Hoyt soon arrives on the scene; he immediately shoots Alex, finds the partially burnt draft card and demands to know which one of the guys is a draft dodger. Eric claims it's his, Hoyt makes them put Alex's body into his car, forces the rest of the group in as well, and calls for Uncle Monty to tow the wreckage. The truck comes and tows the car, in which Chrissie has hidden herself.

Hoyt takes the group to the Hewitt house and calls for Thomas to butcher Alex. He then takes Eric and Dean to a barn and hangs them by their arms from the rafters; he later ties Bailey to the kitchen table. From the car, Chrissie sees her friends tied up and runs back to the highway. She flags down Holden, one of the bikers from the diner, and tells him about Alex and her friends. Holden takes out a revolver and follows Chrissie back to the house.

At the Hewitts', Hoyt wraps Eric's face with cellophane, slowly suffocating him for trying to dodge the draft. Dean begs him to stop and admits the draft card was his. Hoyt shoves a knife into Eric's open, cling-film covered mouth, making him cough up blood, but allowing him to breathe. He then releases Dean and tells him he can walk away free if he can do 20 push ups. As he does them Hoyt brutally beats him with his nightstick. Dean eventually does 10 push ups and Hoyt beats him more, then says he's free to go, although Dean can't move anymore. When Hoyt leaves, Eric breaks free from his restraints and gets Dean to safety while he sneaks back in the house and frees Bailey. As they escape, Hoyt comes outside with his gun and Eric stands in his way while Dean runs, when Dean stands in a bear trap Hoyt takes advantage of the distraction and knocks out Eric. Bailey escapes, taking Monty's truck, but Thomas hooks her in the chest with a meat hook and drags her from the truck. Eric is lying on the kitchen table apparently too injured/weak to move and Hoyt tells Eric "Why do you not want to fight me? Because I'm a pussy? That's what you said isn't it!? Well my nephew Tommy ain't no pussy, maybe you'd like to play with him!", Thomas then carries Eric face up over his shoulder into the basement where he sees Alex's body hanging from the ceiling above the chainsaw, parts of her limbs missing. Thomas punches him, incapacitating Eric, and nails metal straps to a wooden table across his wrists and ankles. Thomas then picks up a knife and cuts open Eric's shirt and twists his head back and forth, inspecting the skin of his face.

Holden enters the house to search for Alex, but ends up shooting Uncle Monty in one of his kneecaps, before being taken hostage by Hoyt. Hoyt leads him to a bedroom and calls for Thomas to help him. Eric now has a strap across his neck as well, and Thomas starts to cut the skin and flesh from his forearm. Chrissie hears Eric screaming and finds the door to the basement, Thomas opens the door and walks past her carrying his chainsaw. Hoyt shows Holden to Bailey, incorrectly thinking this must be the girl he's looking for. Holden points his gun at Hoyt to kill him, Thomas stops him and kills him with his chainsaw. Chrissie finds Eric and tries to free him, but cannot release the metal straps nailed to the table. He says he cannot feel his arm and we see that Thomas has removed all his skin and nerves. Chrissie hides under the table when Thomas comes downstairs with his chainsaw in hand. Thomas inspects Eric's face again then starts up his chainsaw and plunging it into Eric, killing him and cutting right through the table, splattering Chrissie in Eric's blood. He then skins his face, putting it on as a mask. Hoyt calls Thomas upstairs. Hoyt tells Thomas he likes his new face and to chainsaw off Uncle Monty's leg due to the bullet being unable to be removed.

Later Chrissie finds and tries to free Bailey, whom Hoyt has tied up again on the second floor. As Chrissie attempts to untie her friend, Hoyt and Thomas catch her and bring her downstairs for "dinner". There, Dean sits unconscious as Thomas slits Bailey's throat with a pair of scissors, then grabs Chrissie and brings her downstairs as well. On the way to the basement, Chrissie manages to stab Thomas in the back with a screwdriver and escapes by jumping out a window. Realizing their crimes will be discovered, Thomas chases her.

Dean regains consciousness, escaping to the front of the house. There he assaults Hoyt by smashing his head repeatedly into the solid concrete porch in a similar way Hoyt did to Dean and does this because of Bailey's death. He then heads out to find Chrissie. Meanwhile, Chrissie hides in the slaughterhouse. She cuts Thomas' face with a knife she has found, but he violently drags her to the floor. As he is about to kill Chrissie with his chainsaw, Dean arrives to stop him but Thomas impales and kills him with his chainsaw. Meanwhile, Chrissie makes her way to a nearby car and drives off into the night.

Chrissie, desperately looking for help, sees a Deputy who has pulled someone over. As Chrissie is about to pull over, Thomas suddenly appears in the back seat and Chrissie screams. Thomas impales Chrissie in her back through her stomach. Like Eric and Dean, Chrissie dies from the chainsaw. The car, out of control, crashes into the Deputy and a citizen, killing both of them. Thomas exits the car and slowly walks back along the dark road to the Hewitt house, leaving the dead bodies of the Deputy, the citizen, and the young Chrissie. The narrator explains the events and addresses "The Texas Chainsaw Massacre" title.

In the beginning of the film, there is archive footage of a police search of the Hewitt house. The two officers survey the house and descend into the basement, noting the fingernail scratch marks, human blood and hair embedded into the walls.

We are then brought to August 1973 where five young adults, Erin (Jessica Biel), Kemper (Eric Balfour), Morgan (Jonathan Tucker), Andy (Mike Vogel), and Pepper (Erica Leerhsen), are on their way to a Lynyrd Skynyrd concert after returning from Mexico. As they drive through Texas, they see a distraught hitchhiker (Lauren German), who eventually gets in their van. After trying to speak to the hitchhiker, who speaks incoherently about "a bad man", she shoots and kills herself with a .357 Magnum. The group tries to contact the police, then go to a store where a woman (Marietta Marich) tells them the sheriff is at the mill. Instead of the sheriff, they find a little boy named Jedediah (David Dorfman) who tells them that the sheriff is at home drinking. Erin and Kemper go through the woods to find his house, leaving the other three at the mill with the boy. They come to a plantation house where Erin is allowed inside by the owner, an amputee named Monty, to phone for help. When Erin finishes, the old man asks her for help. Kemper goes inside to look for Erin and is killed with a sledgehammer to the head by Thomas Hewitt aka "Leatherface" (Andrew Bryniarski). When Leatherface takes Kemper's body to begin to make a new mask out of him, he hears something hit the bottom of the tub where Kemper is hooked upside down. Leatherface picks up a small black box; opening it, he discovers a ring meant for Erin.

Meanwhile, Sheriff Hoyt (R. Lee Ermey) arrives at the mill and disposes of the hitchhiker's body, wrapping her in cellophane and putting her in his trunk in which he drives away and tells the youths to leave. Erin arrives and finds that Kemper is still missing. Andy and Erin go back to the Monty's house, where Erin distracts him while Andy searches for Kemper. Monty realizes Andy is inside and summons Leatherface, who attacks him with his chainsaw. Erin escapes and heads towards the woods, but Leatherface cuts Andy's leg off. Leatherface carries him to the basement and hangs him on a meat hook with his feet hanging over a piano, where he rubs salt on Andy's stump of a leg before wrapping it in butcher paper and tying it with human hair.

Erin makes it to the mill and tries to escape in the van, but the sheriff shows up and, after finding marijuana, orders Erin, Morgan and Pepper to get out of the van. The sheriff gives Morgan the gun he took from the hitchhiker and tells Morgan to reenact how she killed herself. Morgan, scared and disturbed by the sheriff's demeanor, and under pressure by Erin and Pepper, attempts to shoot the sheriff only to find the gun is unloaded. Sheriff Hoyt handcuffs Morgan and drives him to the Hewitt house (a drive which includes a brutal beating), leaving the girls in the van. Erin tries to fix the truck, while Pepper holds a flashlight.Erin gets the truck running, but the one of the wheels rolls out. Erin and Pepper stay still in the truck but Leatherface appears on the top of the truck and tries to attack them. Leatherface grabs Erin's head and the chainsaw almost impales Pepper. Pepper gets out of the truck, which distracts Leatherface from killing Erin. Leatherface raises his chainsaw, but misses Pepper and cuts a barrel in half by mistake. Erin tells Pepper to run, and she does so. But Leatherface slashes her back and she falls down to the ground screaming. Pepper gets on her back and Leatherface ultimately kills Pepper by cutting her in half. Erin sees that Leatherface is wearing Kemper's face over his own. Erin manages to escape and hides in a nearby trailer with two women inside, who offer her tea and try to soothe her. The two women, a morbidly obese middle-aged woman known only as the 'Tea Lady' and a younger woman named Henrietta, whom is presumably her daughter, act strange and after they tell Erin they don't have a phone for her to call for help, a telephone in the trailer rings and Henrietta picks it up and tells someone on the other end that "she's here". Erin discovers they have kidnapped a child when she sees that the baby with them is the same child in a photograph with the woman who committed suicide earlier in the kid's van. However, the tea is drugged and she passes out when she tries to leave.

Erin wakes up at the Hewitt house surrounded by the Hewitt family: Leatherface, his mother Luda May, Sheriff Hoyt, Uncle Monty, and the little boy Jedediah. Luda May tells Erin that her excuse for her son Thomas's actions, was that her son was tormented by teenagers and that she felt no one cared for her family besides themselves. Erin is taken to the basement, where she finds Andy. She tries to help him off of the meat hook but when he sees he will land on the piano keys and alert Leatherface, he begs her to kill him, which she does, though suffers severe emotional trauma. She finds Morgan, still handcuffed, and Jedediah leads them out of the house. Jedediah rejects Erin's plea to come with them and distracts Leatherface long enough for them to escape. Erin and Morgan find an abandoned house in the woods and barricade themselves inside. Leatherface breaks in and discovers Erin, but Morgan attacks Leatherface, causing him to drop his chainsaw. Morgan grabs him and wrestles him, but Leatherface is too heavy and easily lifts Morgan upwards onto a chandelier before releasing him and Morgan gets tangled in the chandelier by his handcuffs. Leatherface picks up his chainsaw and slices up into Morgan's croch, killing him.

Erin runs out of the shack and escapes through the woods. Leatherface trips and cuts his leg while pursuing her. Erin finds a slaughterhouse and hides in a locker; Leatherface opens the locker across from hers and she attacks him with a meat cleaver, and chops off his right arm. Erin runs outside and flags down a trucker, whom she tries to convince to go away from the Hewitt's house, but he stops to find help at the eatery. Erin sees Luda May and watches as Sheriff Hoyt arrives and talks to the trucker. Erin sees Henrietta watching over the kidnapped baby in a highchair. When Henrietta walks outside to join Luda May and Sheriff Hoyt whom are talking to the truck driver, Erin sneaks the baby out of the eatery and hot-wires the sheriff's car before running him over repeatedly until he is dead. Leatherface appears in the road and tries to stop her, but Erin and the baby escape unharmed.

The police archive footage continues to play. The officers inspect the basement noting the hanging meat hooks when suddenly one of the officers is grabbed and severely beaten. A blurred figure viciously shakes the camera and the other police officer is heard screaming. The narrator states that "The crime scene was not properly secured by Travis County Police. Two investigating officers were fatally wounded that day. This is the only known image of Thomas Hewitt, the man they call Leatherface. The case today still remains open".

In 1974, a woman is making a wax sculpture in the kitchen while her son eats breakfast in his highchair. Her husband enters with another son who is shouting and kicking. The boy is forced into a highchair and strapped in place by his father. After being strapped and taped to his chair by his mother, he scratches her hand. She then slaps her child across the face.

In 2005, Carly (Elisha Cuthbert) and her boyfriend Wade (Jared Padalecki) with her friend Paige (Paris Hilton) and Paige's boyfriend Blake (Robert Richard) are on their way to a highly anticipated football game in Louisiana. Eventually, Carly's delinquent twin brother, Nick (Chad Michael Murray) and his friend Dalton (Jon Abrahams) also join them. Night falls and the group decides to set up camp for the night. Carly and Wade, Paige and Blake make out, but Nick jokingly said to Dalton that he wouldn't kiss him and make love. The campsite is later visited by a stranger in a pickup truck who shines his lights at the campsite, but refuses to leave or address them until Nick smashes a headlight with a bottle. The next morning, Carly and Paige go exploring, Carly falls down a hill and lands in deer remains and sees a fake hand. One of the teen's cars fan belts is found to be damaged. The group meets a disheveled, rural man named Lester (Damon Herriman), who offers to drive Carly and Wade to the nearby town of Ambrose to get a new fan belt, while the rest of them go to the football game.

The two arrive at Ambrose, which is virtually a ghost town. Unable to find an attendant at the auto mechanics shop, they wander into the church, disrupting a funeral. There, they meet a mechanic named Bo (Brian Van Holt), who offers to sell them a fan belt after the funeral. While waiting for the services to end, Carly and Wade visit the wax museum, which itself is made of wax and is the central feature of the town. Afterward, they follow Bo to his house to find a proper fan belt. While there, Wade is crippled and stabbed by Vincent (also played by Brian Van Holt) and Bo grabs Carly, super glues her lips shut and locks her in a cellar. Dalton and Nick arrive in Ambrose to look for Carly and Wade; Vincent meanwhile strips and shaves Wade and then puts him in a chair and puts a metal contraption on his head which has metal pipes sticking into him and pins that keeps his eyes open. Vincent pulls a couple of levers which sends boiling wax that goes through the pipes and shower nozzles which boils Wade in hot wax.

While Nick questions Bo if he has seen his sister, Carly tries signaling for help by sticking her finger out of a vent, trying to get her brother's attention. Bo notices her finger sticking out and leans down by the vent while still talking to Nick and snips the tip of her finger off. Carly falls screaming in pain and later pries her lips open in order to scream for help. Nick, hearing her screams, knocks Bo out after he attacks him. Nick locks Bo out of the station and finds his sister and the two escape. Dalton finds Wade who is alive but he is unable to move or talk because he is immobilized by a layer of wax covering his body. Dalton peels off the wax but realizes he is peeling Wade's skin off. Vincent finds Dalton and accidentally slashes Wade's face with a powersaw. Wade dies from shock. Vincent later chases and decapitates Dalton, killing him. Meanwhile, Nick and Carly realize that the wax figures are actually real people - Bo and Vincent have been trapping people in wax in order for the figures to look more realistic. Bo finds Carly and Nick at the theatres but Nick fires arrows at him which hit his heart but he survives.

Vincent goes to their camp site and kills Blake and chases Paige to an abandoned sugar mill. After being stabbed in the foot, Paige hides in a car but Vincent finds her but she impales him in the cheek with a spear and flees. Paige hides behind a car but Vincent throws a spear through the windows of the car which impales her forehead, killing her instantly. As Vincent brings Blake and Paige's bodies Carly and Nick overhear Bo and Vincent talking and discovers that they were conjoined twins. Nick and Carly find the basement where they find Dalton's body covered in wax. Nick tries to remove the contraption but ends up twisting his head. Bo and Vincent find and chase Carly and Nick to the House of Wax. Nick and Carly unintentionally set the House of Wax on fire. As this is occurring, the wax figures start to melt, as does the whole house. Carly beats Bo to death with a baseball bat, which enrages Vincent. Vincent chases Nick and Carly on to the balcony but Carly and Nick stab Vincent in the hip, thus killing him. As the House of Wax melts from the fire, the room in which all three are in collapses, causing Vincent to land on Bo's corpse. Nick and Carly escape the fiery House of Wax and breathlessly watch as it melts to the ground.

The ambulance and police arrive at daybreak, reporting that Ambrose has been abandoned for ten years when the local sugar mill failed. The policeman (Andy Anderson) reveals that there was a third son of Mr. and Mrs. Sinclair who happens to be Lester. Nick and Carly are then taken to hospital. From inside the ambulance, Carly thought she saw something/someone, so she turns around and looks out through the window, to then see Lester looking and smiling at her.
